CFD: Fire Protection Contractor Permit

For service, repair and/or installation of fire protection equipment pursuant to Chapter 28 of the Chandler City Code, this application must be submitted along with the required documents and payment either by mail or email.

Required Documents

Copies of the following documents must be submitted with your application: 

1. Current contractor’s license (pocket size with the expiration date) for fire protection systems, fire extinguishers and underground fire lines. 
   The following are the licenses we will accept:
   1. Install / Repair / Service Underground  
      1. Accepted Licenses: A , A-12, A-16, C-16, C-37, CR-16, CR-37, CR-80, R-37, or KA
   2. Install / Repair / Service Aboveground, (Fire Sprinklers, Foam, Wet & Dry Chem, Standpipe, Halon & CO2)
      1. Accepted Licenses: C-16, CR-16, or R-16
   3. Fire Alarm & Detection Equipment
      1. Accepted Licenses:
         1. Commercial: CR-11, CR-16, CR-67, C-11, or C-16
         2. Residential: R-11, R-16, CR-11, CR-16, or CR-67
   4. Fire Extinguishers
      1. Accepted Licenses: C-16, CR-16
      2. Install Only: (As Approved by Fire Marshal)
   5. Breathing Air Devices
      1. Accepted Licenses: C-4, C-16, C-37, C-74,or C-77
          
2. Certificate of Liability insurance with Chandler Fire Department as additionally insured.

Permit Fee

• A permit fee of $85 (non-refundable) is due at the time of application. Payments can be made
  • Online at Make a Payment, under Fire Prevention Permits.
  • By cashier’s check, money order, or company check should be made payable to the City of Chandler. 

The Chandler Fire Department Approved Fire Protection Contractors List expires December 31 of each year. You will be required to renew all information each year in January. 

Please remember to leave a copy of the annual service reports with the business owner.
