Commercial Hauler Permit

The permit process is required to be completed each year for all commercial waste haulers wanting to do business in the City of Chandler. Applicants will need to acknowlegde/complete the following, all of which are included in the form below.

Complete Online Forms

Please note that only these documents will be permitted.  No previous versions will be accepted. Do not leave any blanks. 

  • Commercial Waste Hauler Application
  • Commercial Waste Hauler Questionnaire
  • Pollution Form
  • Workers’ Compensation Waiver Form (only if company owners drive their vehicles and have no employees) 

Upload: After completing the forms above, you will be asked to upload copies of:

  • Copy of Insurance Coverage for all vehicles
  • Copy of Maricopa County Permit to Operate (per vehicle)

Pay: Upon completion you will be prompted to pay the following fees online or via check:             

  • Application Fee - $30
  • Vehicle Fee - $1,000 per vehicle

Please submit forms and fees by June 30.
