The permit process is required to be completed each year for all commercial waste haulers wanting to do business in the City of Chandler. Applicants will need to acknowlegde/complete the following, all of which are included in the form below.
Acknowledge that have read and understand the following:
- Chandler City Code, Chapter 44
- Insurance Coverage Requirements
- Arizona Revised Statute - 28-1098. Vehicle loads; restrictions; exception; civil penalties.

Please note that only these documents will be permitted. No previous versions will be accepted. Do not leave any blanks.
- Commercial Waste Hauler Application
- Commercial Waste Hauler Questionnaire
- Pollution Form
- Workers’ Compensation Waiver Form (only if company owners drive their vehicles and have no employees)
Upload: After completing the forms above, you will be asked to upload copies of:
- Copy of Insurance Coverage for all vehicles
- Copy of Maricopa County Permit to Operate (per vehicle)
Pay: Upon completion you will be prompted to pay the following fees online or via check:
- Application Fee - $30
- Vehicle Fee - $1,000 per vehicle
